INFORMATION CLAUSE
1. The administrator of your personal data is the University of Warsaw with its registered office at 26/28, Krakowskie Przedmieście, 00-927 Warsaw, Poland.
2. University of Warsaw Data Protection Officer supervising the accuracy of personal data processing can be contacted by e-mail: iod@adm.uw.edu.pl.
3. Your personal data will be processed in order to conclude the contract with the data subject, or to take action on request of the data subject before concluding the contract or fulfillment of legally justified objectives pursued by the University of Warsaw.
4. Data provided will be processed according to Article 6 section 1 item a or b or f of the General regulation on personal data protection (Official Journal of the European Union no L 119, 4.5.2016) 2016/679 as of April 27, 2016.
5.  Providing your personal data is not obligatory but is a requirement for the fulfillment of purposes for which the data is collected.
6. Data will be disclosed to third parties in the cases referred to by provisions of law and under concluded agreements for the outsourcing of data, and will be communicated to entities providing services for the University of Warsaw such as: IT services and systems providers, payment operators, postal operators, delivery companies, and other subcontractors.
7. Data will be stored for the period necessary for the execution of the stipulations of the sales contract and for the time necessary due to regulatory obligations in accounting and tax settlements. Data processing is based on your consent and hence data will be processed until withdrawal of consent. Consent may be withheld at any time upon requesting withdrawal via e-mail: wuw@uw.edu.pl.
8. You are entitled to access your personal data subject to the provisions of law: the right of data correction, data erasure, limitation of data processing, the right to data portability, the right to object, the right to withdraw consent at any time.
9. You are entitled to file a complaint to the President of the Office for the Protection of Personal Data (PUODO).
10. Your personal data will not be processed in an automated way, nor profiled.
